We strive to provide affordable options and offer various payment methods, including cash, check, credit card, and CareCredit® for financing. Office Policies
Cancellation and Missed Appointment Policy
Your appointment time is reserved specifically for you and for you only. Because of this, missed appointments or late cancellations are extremely detrimental to our day. As a result, we request at least 2 business days advance notice if you will not be able to make your appointment. If required notice is not given, your appointment is considered a missed appointment. Missed appointments will be assessed a fee of $75 per half-hour of the scheduled appointment length. This fee is due by the patient.
After 2 missed appointments, you will be on a walk-in basis only, meaning you must contact us when your day allows for an appointment and we will see you if the schedule allows. Repeated missed appointments or late cancellations may result in fees or dismissal as a patient.
Financial Policy
Payment in full for your treatment is due no later than when services are rendered. Acceptable forms of payment include cash, Visa, Master Card, American Express, Discover, and assigned insurance benefits. Credit cards are subject to a 2.99% surcharge paid by the patient. If you wish to avoid this surcharge you may use cash, check, or debit card as payment.
In the event there is a shortage due to insurance underpayment, it is our policy to send a digital statement. The outstanding patient balance is due no more than 30 days after you have been notified of a balance due. Once notified, any balance may be automatically applied to the credit card on file.
Unpaid patient balances over 90 days old will be subject to a monthly interest of 0.75% (9% yearly maximum) per Washington State law RCW 19.52.020. If payment is delinquent, debt will be transferred to a collections agency or pursued in small claims court. The patient will be responsible for payment of collection, attorney’s fees, and court costs associated with the recovery of the monies due on the account. Payments returned due to non-sufficient funds will be subject to a NSF fee of $25.00.
